Proof of Insurance

Proof of insurance must be provided at least 15 days prior to the event listing Florida Sports Foundation as an additional insured with a minimum liability of $1,000,000 per occurrence.

 

 

You can find your Grant Application ID on the email you received from Florida Sports Foundation when you originallty submitted your application.
