Supporting Documentation for Post-Event Reporting
Post-Event reports are available in the Fluxx Grant Portal (the same portal where the application was submitted). Post-Event Reports are due within ninety (90) days of the completion of the event. Extensions can be requested in writing, on a case-by-case basis.
Certificate of Insurance
An insurance document listing:
- Florida Sports Foundation as an additional insured for all purposes
- Minimum liability coverage of $1,000,000 per occurrence
- Coverage active during the occurrence of the event
Expense Reimbursement
For reimbursement of an expense, the following are required to be attached to the Post-Event Report. Please note that the below is required for each expense submitted.
- Proof of payment (e.g., cancelled checks, EFT or ACH payment details with the date the funds were delivered) and must include:
- Payee Name
- Payor Name
- Date the transaction was processed
- Invoice or other detailed explanation of the expense
- Vendor Name
- Customer Name
- Event Name (or reference to)
- Must contain an explanation of the expenditure
- Must be on the list of allowable expenses
- Both the proof of payment and the expense detail must reflect the same names to clearly demonstrate they are related.
Attendance & Hotel Data
This section of the report provides the actual number of participants, attendees, and room nights. The origin of each report should be provided as well.
- Proof of Participants
- Registration reports listing the number of participants
- Rosters of coaches, officials, etc.
- Economic Impact studies reflecting actual (not estimated) numbers
- Proof of Hotel Nights
- Hotel Pickup reports
- Economic Impact studies reflecting actual (not estimated) numbers
Florida Visibility Promotion
Supporting documentation that refers to the promotion of the state of Florida, the Florida Sports Foundation, and the professional sports and specialty license plate program includes:
- URLs and screenshots from social media postings*
- Digital photos of logo and banner placement*
- Digital copies of program advertising
- URLs or digital recordings of any broadcast (television or internet)
